Phase I:

Submission of New Student Interest Form with $15.00 Application Fee
Interview with Program Director
Application for Enrollment, Parts I through IV
Current Student Health form
Parent Commitment Contract
All applicable fees
1/10 tuition amount (1st month’s tuition)
Former School records including any standardized test results, psychological evaluations, etc.

Phase II: (At orientation)

Submit completed SMART enrollment form
Submit completed tuition contract
Pay early NH registration fees
Pay AYO registration fee, $25
Pay first tuition amount, including AYO
